Incident Report with Custodian Robert Zykewski
I was mopping up after lunch when he started following me. I thought he just wanted to watch me work. You know a lot of them do that. But he started acting nervous. He was scared or something. I got real nervous so I turned my back on him as soon as I did he swiped at me. Tore my uniform too. I told the boss it was that bad but he made e pay for a new one. Anyways he orderly came and took him back to his room. I think they locked him in there. -
Incident Report for Resisted Curfew
Staff Report: Patient resisted curfew this evening. Orderly could not physically move patient from hallway into his room for the night. Confrontation escalated and patient shouted "bed" several times before calling out for his Mother. A second orderly arrived to assist and the patient struck the second orderly in the side of the head with his fist. This is the second incident of a violent nature in a month's time. Incident Report of Muttering To Himself
Patient was in exercise yard today when he began muttering to himself. Other patients and staff reported that among the muttering they heard him say the following words and phrases: never be good, blew him up, said I can't do it, Dad now, blue, and special. Our nurses and orderlies observing him reported that his behavior was upsetting several of the other patients in the vicinity. Lilian Grayson Death at 2300 Hours
Lilian died at 2300 hours (11:00pm). COD: blunt force trauma to the head. Also had 13 stab wounds to the abdomen, 2 stab wounds to the right hand, 1 stab wound to neck, collapsed orbit, fractured skull, effusion of blood in vagina, and inflammation of vulva. She also had heightened levels of progestin and estradiol in her blood. -
